Work closely with other healthcare professionals to ensure comprehensive care coordination.
Monitor patient outcomes and adjust dietary plans as needed to achieve optimum results.
Educate patients and families on nutritional management of their specific conditions.
Document assessments and plans in patient records while adhering to confidentiality and HIPAA regulations.
Stay current with latest research in nutritional science to provide evidence-based care.
Strong knowledge of dietary management and clinical nutrition.
Ability to communicate effectively with patients, families, and healthcare team members.
Expertise in data analysis for health management and outcome prediction.
Skill in developing and conducting educational programs in nutrition.
Current Registered Dietitian (RD) and Certified Nutrition Support Clinician (CNSC).
